Your complaint with the airline regarding denied boarding, cancellation or long delay of flights must be lodged using the enclosed complaint form. (The complaint form cannot be used for lodging complaints regarding the loss, delay or damage of your lugagge.)

Dear passengers,

your complaint with the airline regarding denied boarding, cancellation or long delay of flights (on the basis of the Regulation (EC) No. 261/2004 of the European Parliament and of the Council) must be lodged using the enclosed complaint form.

Fully completed complaint form should be sent to the airline company. In case the airline company has failed to provide you with the reply within a period of 6 weeks from the date of recieving or if you are not satisfied with the reply given to you, please, fill in and send this completed form 
to the Civil Aviation Authority of the Czech Republic (caa@caa.cz or podatelna@caa.cz).

The complaint form cannot be used for lodging complaints regarding the loss, delay or damage of your luggage.
